The ony exception to this rule in making a given severity rating is when the intensity rating is two or more steps above the frequency rating - in such situations, you can make a severity rating that is one step higher than the frequency rating.

You may make a severity rating of Moderate/threshold if a symptom occurs 1x/month (lower than the required 2x/month) as long as intensity is rated Pronounced or Extreme (instead of the required Clearly Present).

Severe/markedly elevated: 1x/week

Similarly, you may make a severity rating of Severe/markedly elevated if a symptom occurs 1x/week (lower than the required 2x/week) as long as the intensity is rated Extreme (instead of the required Pronounced). If you are unable to decide between two severity ratings, make the lower rating.
