Here you will enter the severity score for each symptom under the "sev" column (range 0-4). Transfer each symptom’s severity score to this page. If the trauma-relatedness (TR) of an item was "unlikely," list the severity as "0," regardless of what severity score you gave.
Sx (Sev ≥ 2 )?
In the "Sx (Sev≥2)" column, you will indicate whether each symptom should count towards the PTSD diagnosis by circling “0=No” or “1=Yes.” A symptom is considered present only if the corresponding item severity score is rated 2 ("moderate/threshold") or higher and TR is definite or probable.
Criteria B Subtotals
For each PTSD symptom cluster, you will indicate the total severity score and the total number of symptoms from that cluster that can be counted toward a PTSD diagnosis. For the former, simply add the severity scores for each item in the cluster (i.e., the “sev” column). For the latter, add the number of symptoms that you scored as “1=Yes” in the “Sx (Sev≥2)” column.

An interviewee's total severity score on the CAPS-5 represents the sum of the individual severity scores (0-4) for each of the 20 PTSD symptoms (CAPS-5 items 1-20 that have TR definite or probable). Total scores range from 0-80, with higher scores indicating more severe PTSD symptoms.
